Preparticipation Examination Forms
In order to be prepared for your arrival on campus, we ask that you complete all forms pertaining to your year of athletic eligibility (Incoming Freshmen/Transfer or Returning Athlete). Please return those forms in an addressed stamped envelope to the following address:

These forms will provide us with the pertinent information related to your medical history, which will allow for a more thorough and efficient physical examination. Each of the following forms and documents must be completed and returned to the Southeastern Louisiana University Sports Medicine Department by all student-athletes.
